POLICY & PROCEDURES
I strongly believe that your time is valuable as is my time. I will make every attempt to be ready and on time for your scheduled appointment. To provide you with an outstanding experience to all of my clients and out of consideration for your time and mine, I have adopted the following policies:Arrival to your massage
Please arrive for your appointment 10-15 minutes prior to the scheduled starting time. This will allow you the time to fill out appropriate client forms (NEW CLIENTS), change and prepare for your session. All massage sessions have a specific time schedule and early arrival allows for a relaxed unhurried experience. If late arrival, your service may be shortened in order to keep on schedule and the original treatment time will be charged.Cancellation Policy and Fees
Your therapy times are reserved exclusively for you. I value your business and ask the you respect the scheduling policies. I know unexpected events happen in our lives, car problems, children’s illnesses, work schedules changing are some reasons you may need to cancel a massage appointment. However, I ask that you call or text me if you cannot keep your appointment. In my efforts to be effective and fair to all my clients, I ask that the following policies be honored:• 24 hour advanced notice required when canceling an appointment. This allows the opportunity for someone else to schedule an appointment.
• If you are unable to give a 24 hour advanced notice know that you may be charged the full amount of your scheduled appointment.
• Fees will be waived in cases of contagious illness, if you are sick please stay home and rest, we can reschedule for when you are feeling better.Clients who miss their appointment without giving any prior notification will be considered a no show and will be charged the full amount for their missed appointment.Informed Consent
Before each massage session, the treatment plan will be discussed with you. At your first visit with me you will be asked to sign the treatment consent form stating that you have read all the information, understand it, and agree to comply with me your professional massage therapist. Clients who have not been seen for at least 6 months may also be asked to fill out this form.Scope of Practice
I Nora Baima, I am a Illinois licensed professional and am held to the highest standard of the American Massage Therapy Association.Massage Therapy is a profession on in which the practitioner applies manual techniques with the intention of positively affecting the health and wellbeing of the client. As a Licensed Massage Therapist, I do not diagnose or prescribe for medical conditions. I may refer you to a medical doctor for diagnosis or treatment. My services are not intended as an alternative medicine. Please refer to your medical doctor for any specific condition which requires medical treatment.Clients’ Needs and Boundaries
I am always happy to adjust pressure, temperature, music, music volume, work longer on an area or move on if you request. Please communicate anything that will make your session more enjoyable.The client may choose to: Leave on as much clothing as needed for comfort, refuse any massage methods, stop the massage at any time.The client will always be modestly draped. ONLY the areas that are being massaged will be undraped. The clients will be kept informed of the area to be massaged.Sexual interaction or discussion of any kind between the client and the massage therapist is NEVER appropriate and will cause termination of the session.Confidentiality and Conversation
I treat all my client visits and conditions as confidential. As such, I will not discuss your visit with anybody outside (your physician team) without your permission.You may choose to talk, not talk during the massage session. Conversation will be guided by you the client.Existing and New Medical Conditions
It is you the client’s responsibility to keep me your Massage Therapist informed of any medical treatment currently being taken, and provide written permission from the physician, chiropractor, physical therapist etc. that the massage may be continued.You must also keep me informed of any changes in the health conditions.For clients undergoing chemo and radiation therapies- Please note that we require a doctor’s note that states the doctor is aware of and agrees to the desired treatment.If you have and questions on the above information please direct them to me.Thank you,
